- Patients are admitted on a non-dialysis day
- Admission RN will perform a comprehensive assessment
- Assessments for Physical Therapy, Occupational Therapy
- Fluid Management
- MSW and Home Health Aide
- Education provided on disease process and lifestyle modifications
- Assessment for infection risks
- Assess psychosocial needs (i.e. Behavioral Health Program)
- Assess for abnormalities post dialysis treatment
- Medication management and medication reconciliation performed
- OT focus on energy conservation
- Physical therapy focus on endurance
- Provide/Assist with wound care as needed
- Provide/Assist with insulin injections to diabetic patient as needed
- Assist with infusion therapy on non-dialysis days
- Maintain coordination of care with dialysis center for changes in patient’s condition as needed
- Timely clinical updates to physicians as needed
