Cardiac Care Services

For patients diagnosed with High Blood Pressure, Congestive Heart Failure (CHF), Cardiomyopathy, Transplants, Coronary Artery Disease, Stroke, Heart Attack or Peripheral Vascular Disease.


The Roads Home Health’s nurses are available to assess chest pain, heart and lung sounds, respiratory difficulties, and wounds to provide the necessary physician-ordered care. Our cardiac care services provide patients with disease specific education of symptoms and medication management to achieve the outcomes needed to reduce hospitalizations, re-hospitalizations and frequent ER visits due to exacerbation of symptoms. Using standardized educational programs, patients are taught healthy heart living, how to care for themselves following cardiac procedures and surgeries, and are given knowledge about the cardiac drugs that have been prescribed. Care is directed by the patient’s physician and brought to the patient through highly trained and experienced cardiovascular nurses. To keep cardiac patients heart-healthy at home, they provide:

  • Cardiac assessment and weight monitoring
  • Infusion treatment at home
  • Pulse oximeter testing
  • Point of care PT/INR testing
  • Patient education specific to symptom management of CHF and prevention of exacerbation
  • Falls prevention identification and education
  • Diuretic therapy and electrolyte replacement as ordered by MD
  • Dopplers for patients with LVAD’s & medication management
